Scalp Psoriasis

Also called: psoriasis capitis

Scalp psoriasis is a chronic autoimmune skin condition characterised by thick, silvery, adherent plaques on the scalp. It affects up to 80% of patients with systemic plaque psoriasis and can occur in isolation.

How it happens

Psoriasis is driven by a dysregulated T-cell–mediated immune response (Th1/Th17 axis) that accelerates keratinocyte proliferation (turnover in 3–5 days vs normal 28 days), producing the characteristic thick scale. Unlike sebderm, the scale is silvery-white, micaceous, and well-demarcated, and may extend beyond the hairline as a "corona psoriasis".

In India

Psoriasis affects 2–3% of the Indian population. Scalp involvement occurs in approximately 80% of psoriasis patients.

Key symptoms

  • Thick, silvery-white plaques on scalp
  • Well-demarcated borders extending beyond hairline
  • Intense itch - may disturb sleep
  • Koebner phenomenon - plaques appearing at sites of trauma
  • Associated nail pitting, onycholysis
  • Possible arthritis (psoriatic arthritis)

Common triggers

  • Genetic predisposition (HLA-Cw6)
  • Streptococcal throat infection
  • Medications (lithium, beta-blockers, antimalarials)
  • Psychological stress
  • Alcohol excess

How it is diagnosed

  • Clinical examination - Auspitz sign (pinpoint bleeding on scale removal)
  • Dermoscopy - dotted vessels, white scale
  • Scalp biopsy (if diagnosis is uncertain)
  • ESR, CRP (if joint involvement suspected)

Tests are performed by a qualified dermatologist or trichologist after clinical assessment.

Treatment options

Potent Topical CorticosteroidsEvidence A

Clobetasol or betamethasone scalp lotion - most effective first-line agent for scalp psoriasis.

Calcipotriol + Betamethasone GelEvidence A

Combination product - steroid reduces inflammation while Vitamin D analogue reduces proliferation.

Coal Tar ShampooEvidence B

Anti-proliferative and anti-pruritic. Useful as maintenance or combined with topical steroids.

Systemic Biologics (IL-17, IL-23 inhibitors)Evidence A

For moderate-to-severe or refractory disease. Requires specialist referral and monitoring.

Timeline: Topical treatment produces visible improvement within 4–8 weeks. Biologics can achieve near-complete clearance within 12–16 weeks.

When to see a doctor

All patients with suspected scalp psoriasis should be formally evaluated by a dermatologist - systemic treatment options have transformed outcomes in moderate-to-severe disease.

Dermatologist perspective
“Scalp psoriasis is commonly underdiagnosed or mismanaged as simple dandruff. The key differentiator at dermoscopy is twisted, dilated capillary loops in a regularly distributed pattern - distinctive from sebderm's arborising vessels.”
